Tanya Bish 

Head of Clinical Delivery, Summerset Group Holdings
Tanya Bish is a New Zealand-trained nurse with a career spanning both public and private aged care sectors. As Head of Clinical Delivery at Summerset Retirement Villages, she leads clinical service delivery across a national network of retirement communities.
With a Bachelor of Commerce and a Master of Nursing, Tanya brings a unique blend of clinical leadership and operational insight. Her work is deeply rooted in promoting the health and wellbeing of older people, with a particular focus on resident-directed care.
Tanya is passionate about developing equitable, sustainable models of care that uphold dignity, foster connection, and improve quality of life for residents and their whānau. Her leadership is informed by a commitment to cultural responsiveness and continuous improvement.

Seamless Aged Care Services​

Over the past four decades, the aged care sector has undergone significant transformation, shaped by demographic shifts, evolving consumer expectations, workforce challenges, and more recently, the global Covid-19 pandemic. These pressures have accelerated the need for digital innovation, exposed systemic vulnerabilities, and highlighted the importance of integrated, responsive care models.
Despite these changes, many of the sector’s constraints remain persistent: fragmented service delivery, funding limitations, skilled workforce shortages, and inequitable access—particularly for rural, Māori, Pacific, and other diverse communities. As our population ages and becomes increasingly diverse, the urgency to create sustainable seamless service provision grows.
 
This session will explore what it takes to create sustainable, seamless services that are:
•          Person/whānau directed
•          Locally accessible: Supporting whānau inclusion
•          Digitally enabled: Leveraging technology to improve coordination, communication, and outcomes
•          Workforce-supported
 
We will examine practical improvements needed to achieve seamless service delivery, including:
•          Integrated care pathways that reduce duplication and improve transitions between health, social, and aged care services
•          Organisational culture changes
•          Co-design with older people and whānau, ensuring services reflect lived experience and local context.
 
Ultimately, this session will challenge us to think beyond traditional models and ask: What does good look like in aged care—and how do we get there together?
